Care Manager

LIFEPlan Care Managers assist people with intellectual and or developmental disabilities through the person-centered planning process to identify a person’s priorities, goals, and dreams. With the person, and their circle of support, the Care Manager will coordinate the creation of an individualized Life Plan (LP) that includes supports and services as needed. The Care Manager will handle all care coordination – e.g., monitor, assess, and work with the person and the support team to integrate supports and services, making changes as the person’s needs change, through:
  • Providing comprehensive care management, including the development of a Life Plan, using care coordination and promoting a healthy lifestyle.
  • Engaging the person and family and/or representative to understand concerns and what is needed in the transition process; taking an active role in discharge planning.
  • Ensuring that persons and families and/or representative, with cultural sensitivity, have the information and or services needed to maintain and promote quality of life.
  • Assisting the person in becoming part of the community, not just in the community, to discover new and or meaningful, community experiences that enrich his/her life.
  • Advocating by ensuring the person is in the center of determining his/her desired outcomes and promoting self-advocacy and a person’s civil rights.
  • Leading a person through the process of registering for Medicaid services and ensuring that individuals have access to Waiver Medicaid services. Ensuring eligibility for Medicaid and all other applicable services (Behavioral, Medical, HCBS etc.) is maintained.
  • LIFEPlan Care Managers also will:
  • Adhere to HIPAA, confidentiality, and other privacy standards determined by LIFEPlan, OPWDD, DOH, etc.
  • Ensure required training hours are completed.
  • Be required to use their personal vehicles for frequent face to face, in-home visits.
  • Must have NY State Driving License
  • Manage other duties as assigned.
QUALIFICATIONS:
Education/Work Experience:
Bachelor’s degree required – in Health and Human Services, Psychology, Sociology, or related fields preferred, plus two years relevant work experience required – ideally with people with disabilities, in case management, or in the Mental Health or Substance Abuse, chronic conditions or other related conditions. Relevant experience is considered full-time or part-time paid or volunteer experience that consists of providing direct care or support, coordination, and/or advocacy for any person on a regular basis.
The following experience with any group includes, but is not limited to:
  • Direct care, Home care or Habilitation;
  • Daycare or child care;
  • Teachers, teacher assistants, or teacher aids;
  • Case management;
  • Experience working in an alternative school setting (e.g. BOCES, Questar);
  • Counselors; or
  • Clinical/medical field (e.g., Physical Therapy (PT), Occupational Therapy (OT), Speech and Language Pathology (SLP), Master of Social Work (MSW), Licensed Clinical Social Worker (LCSW), Nursing including Certified Nursing Assistant (CAN), Licensed Practical Nurse (LPN), and licensed technicians such as x-ray or ultrasound).
Or, individuals qualify if they have the following experience specifically working with individuals with intellectual and developmental disabilities (I/DD).
  • Parents or family members of a person diagnosed with I/DD; or
  • Internships (including those required for education).
Licensed Registered Nurse with two years of relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, or
Master’s degree with one year of relevant experience.
Other skills and abilities include: Listen actively. Use required technology. Discuss issues with members of multiple disciplines. Apply knowledge of the community/state resources, I/DD, SSI/Medicaid eligibility, funding sources, health and safety needs. Respectfulness. Rapport building with all involved in the Life plan. Maintain composure under pressure. Problem solve and make informed decisions. A valid NY State License is required.
